21 Together Participation Form

If you are registering for attendance at Speech Clubs please also complete the following Personal Care Declaration Form.

Please ensure children/young people have used the toilet facilities before starting their sessions.

We intend to have regular set times where the whole group will be asked and encouraged to use the toilet. In these times, at least two adults will be present in the area to supervise and assist if needed. However, we anticipate that some participants may need higher levels of assistance or may need help at other times. Please complete this form and discuss any concerns with a member of the 21 Together team.
